The CHCP provides or arranges a wide range of services to help people live as independently as possible in their own homes and communities. People who need community care services may be older people who need support to live at home, adults with physical disabilities, learning disabilities, mental health problems, substance misuse problems, sensory impairment or someone leaving hospital or requiring a care home placement. CHCP staff aim to provide support tailored to individual needs.  

We work in partnership to provide best value, high-quality services to those who need them the most. The CHCP will lead on improving outcomes for service users through the further integration of services across all community care groups.
