Health Improvement Team

Improving health and well-being and addressing health inequalities in East Renfrewshire are key priorities for the Community Health and Care Partnership

Health improvement is an outcome - it involves all communities and individuals.

The CHCP Health Improvement Team plays a leading role in supporting partner agencies and communities to ensure that improving health and reducing inequality is incorporated into local plans, public services and community activity.

We use a combination of approaches including:

  • Preventing ill health (sometimes called ‘primary prevention’)
  • Treating physical and mental health issues (secondary prevention)
  • Reaching people who are most at risk of physical and mental ill health
  • Reducing barriers to ensure local services recognise peoples needs
  • Integrating health improvement into other local services and plans

This involves

Delivering health improvement services and activities related to:

  • topics e.g. smoking, oral health, youth, men’s health, physical activity, healthy weight, infant feeding including breastfeeding, alcohol and mental health
  • Settings - e.g. nurseries, schools, workplaces and communities
  • Life stages - e.g. early years, youth and older people

Providing knowledge and expertise within partnerships to ensure health improvement is incorporated into high level strategies and plans such as the Single Outcome Agreement and Children's Services Plan.

Building community capacity through training and research to ensure communities can influence services and policies that impact upon their health.

Developing workforce knowledge throught the Health Improvement Network

Providing development support to the Public Partnership Forum
