Older People's Healthcare

Eastwood area

Eastwood Community Older People’s Team

Provides services for people of 65 years and over for people who are frail, becoming housebound, at risk of falling, or, are losing the ability to cope safely at home. Team membership consists of District Nurses, Physiotherapists, Occupational Therapist, Social Worker, Dietician and Rehabilitation Assistants. They also have close links with the Community Mental Health and Podiatry Teams.

Referrals can be made for a variety of reasons, for example:

  • Crisis referrals to prevent admission to hospital
  • Rehabilitation plans to improve function and quality of life following illness or general decline in physical/mental health.
  • Referrals are accepted from General Practices and other health professions, Social Work, Housing and other community services with the agreement of the older person.

Eastwood Older People’s Community Mental Health Team

They provide a comprehensive Mental Health Service for people aged 65 plus, (or younger if there is a diagnosis of a progressive organic disorder e.g. dementia).

The team provides access to the following services:

  • Community Psychiatric Nurses
  • Occupational Therapy
  • Consultant Psychiatrist
  • Psychology

Areas served: Busby, Clarkston, Eaglesham, Giffnock, Muirend, Netherlee, Newton Mearns, Stamperland and Thornliebank

  • Referrals are accepted from medical staff, health professionals and social work staff

Levern Valley area

The Levern Valley Older People’s Team

They provides a responsive service for people 65 years and over, working with frail elderly and mental health clients. The Team is made up of District Nursing, Psychiatric Nursing, Social Work, Physiotherapy, Occupational Therapy and Older People’s Team Assistants.

Referrals can be made to the team for a variety of reasons:

  • Crisis or Urgent referrals to prevent admission to hospital
  • Rehabilitation referrals, with rehabilitation programmes lasting approximately 16 weeks or longer if required
  • Support early hospital discharge for patients with complex needs

Areas served: Barrhead, Neilston and Uplawmoor.

  • Referrals are accepted from medical staff, health professionals, social work staff and community groups via Single Shared Assessment
